We deliver effective psychiatric rehabilitation services in the client’s natural environment/or community.
- Decrease the prevalence of psychiatric and emotional disabilities in adults and families.
- Consumers receive rehabilitation services from their own rehabilitation coordinators and therapists who they know and trust.
- Work collaboratively with other community and state-based mental healthcare providers to achieve consumer’s goal and maintain lasting recovery
- Encourage independence and self-sufficiency

- To create a rehabilitation plan that compliments our client’s goals currently in place

- Providing ways to manage illness/Psychosocial education,

- Helping client learn socialization skills both at home and/or vocational/work settings,
- Learning Community integration skill after a brief of long-term placement in a restrictive setting,
- Daily Activity skills to increasing work readiness/prevocational skills and participation in a meaningful activity outside of the home; and
- Money Management skills and proper use of leisure hours to prevent substance used or dependency.
- Team K Behavioral Healthcare Services will develop a partnership with community-based agencies to offer or make referrals for specialized interventions not offered by the agency. Team K Behavioral Healthcare Services intends to offer a wide variety of Psychiatric Rehabilitation Services to children and adults with psychiatric or emotional problems.
The goals, objectives, and expected outcomes, including a projection of the number of persons served expected to transition from one program to another.
Team K Behavioral Healthcare Services will adopt the following goals as guiding principles in the delivery of quality and medically necessary Psychiatric Rehabilitation Services:
- Provide self-directed recovery intervention as the foundation of a psychosocial delivery system
- Use the screenings, comprehensive psychiatric assessments to determine the individual’s needs, strengths, and ability to function independently in the community.
- Allow clients, family members, and referral sources to become an integral component of the planning, delivery, and ongoing evaluation of PRP interventions offered.
- Ensure that individuals receiving services are treated with respect, empathy, and informed in a responsible manner.
- Ensure that all services are delivered in a professional and ethical manner using up to date evidence-based practices in the recovery-focused treatment of individuals with emotional disabilities
- Provide psychosocial intervention in the least restrictive, most integrated community setting that is warm, inviting, and respectful of the clients.
Team K Behavioral Healthcare Services will work with the Behavioral Health Administrative Service Organization (ASO) to determine the level of care needed by each client referred and in transferring the clients with critical psychiatric needs to other agencies or discharging those who have successfully complete their objectives/goals/IRP, ability to live in the community independently and overall mental illness has improved.

The expected outcomes for Team K Behavioral Healthcare Services, Psychiatric Rehabilitation Services will include:
Increased community living success of clients
- Reduction in the number of hospitalizations
- Reduced involvement with criminal justice system/ juvenile system
- Increased active participation in treatment and PRP services
- Improved vocational functioning
- Improved educational status
- Improved stability of housing status
- Decreased use of illicit and/or alcohol
- Compliance with mental health services, including taking prescribed medication by the psychiatric or medical personnel.
- A reported increase in the quality of life.
Team K Behavioral Healthcare Services will ensure that the outcome measure will be incorporated into all aspects of the Psychiatric Rehabilitation Program (PRP)delivery system at the agency. Each staff member will be involved with the process of monitoring and assessing how these outcomes are met weekly, monthly, and quarterly in providing PRP services to individuals.
