Team K Behavioral Healthcare Services, Psychiatric Rehabilitation Services will target population clients who meet the medical necessity criteria set by and have or are willing to apply for Medical Assistance and are cognitively capable of actively participating in their request for admission. Our target population is unserved, disfranchise communities and senior citizen communities who are overlooked. Team K Behavioral Healthcare Services will assist the person served who has been impacted by social determinants and systemic racism that has impacted the client’s mental health. Team K Behavioral Healthcare Services plan to serve adults and children with chronic mental health issues who need support in developing or maintain the skills needed to function successfully in the community. Person Served will continue to receive clinical mental health services throughout their enrollment in our program. Team K Behavioral Healthcare Services will include the following population of persons served:

Transitional age children and adolescents (16-21 years) who are wards of the state and have a chronic history of emotional disabilities and without PRP service make fall through the crack in returning to the community.

  • Client with chronic psychiatric illness and are homeless, on probation or parole, and history of recidivism.
  • A client returning from a state institution with a chronic history of disabilities and poor community living skills.

Team K behavioral Healthcare Services will work closely with Baltimore Mental Health System to identify critical areas of need given the declining resources for community mental health services.
In the first operation, Team K Behavioral Healthcare Services intends to provide services to 25-30 Adult/Minor clients in Baltimore City after receiving preauthorization from the Behavioral Health Administrative Service Organization (ASO). Team K Behavioral Healthcare Services will maintain an active list of proper credentialed Psychiatric Rehabilitation staff members to ensure that no client referred for PRP services is placed on a waiting list or denied services.
